- CGHS token card
- Undertaking from parents that the child is not suffering from chronic diseases like epilepsy,depression, major psychosis, tuberculosis etc.
- Vaccination report against the typhoid fever.
- Report of haemogram and urine examination.
- For NCC and Admission to colleges Eye specialist consultation for refraction and colour blindness.
